Q:

For the goals you do not set in Section GG, should you use a dash or an “09” Not Applicable? Won’t dashes affect the QRP?

A:

A dash may be used in column 2 of section GG for any goals that are not set, and not affect the QRP as long as at least one goal is coded and there is a care plan to address it.  While the use of the “09” Not Applicable code is permissible in column 2, it should not be used for goals that are not set. The code of “09” Not Applicable is only used if the activity was not attempted and the resident did not perform this activity prior to the current illness, exacerbation, or injury.
