Q:

I have a resident that routinely receives an injection of fluphenazine decanoate every 4 weeks. The injection was not administered during the 7-day observation period, but its a long-lasting medication. Should I code it under N0415A?

Page N-9 of the RAI manual includes the following coding instructions for N0415A1:

“Count long-acting medications, such as fluphenazine decanoate or haloperidol decanoate, that are given every few weeks or monthly only if they are given during the 7-day lookback period (or since admission/entry or reentry if less than 7 days).”

Therefore, you would not check the box for N0541A, but you would code it under item N0450A: “Did the resident receive antipsychotic medications since admission/entry or reentry or the prior OBRA assessment, whichever is more recent?”
