When is a Gradual Dose Reduction (GDR) required for antipsychotic and psychotropic medications in a Skilled Nursing Facility (SNF)?

 

A:

For a resident who is receiving an antipsychotic, a GDR is required unless it is clinically contraindicated. An attempt to reduce the dose should be made during the first year of the resident’s admission or following the initiation of the antipsychotic. This attempt must occur in two separate quarters with at least one month between attempts, and then annually thereafter unless clinically contraindicated. (Reference: Form CMS-20082)
