How often do restorative progress notes need to be written?

 

A:

The RAI Manual, p. O-43 states, “Evidence of periodic evaluation by the licensed nurse must be present in the resident’s medical record.”  Facility policy should determine the frequency of the evaluation (i.e. weekly, monthly).  However, if your facility is in a case mix state, refer to the supportive documentation requirements guidelines user guide also for your state.  For example, Indiana’s guide, p. 27 specifies, “For the case mix review, reassess progress, goals and duration/frequency of each program within the observation period.”
