The SNF FY2022 Proposed Rule included two measures that will be solely based on the patient and/or family’s report. These include the Patient Reported Outcomes Measurement Information System (PROMIS) and the CoreQ Short Stay Discharge Measure. CMS has not been specific as to which PROMIS tools they will utilize to measure functional outcomes, but you may find the PROMIS website helpful to evaluate the patient’s perception of physical, mental, and social health. Representing the patient’s experience of care, the CoreQ calculates the percentage of individuals discharged in a six-month time period from a SNF, within 100 days of admission, who are satisfied. This patient reported outcome measure is based on the CoreQ: Short Stay Discharge questionnaire that utilizes four items.
