My IDT team determined one of our residents needs a Significant Change assessment completed. I set the ARD, is it correct that I have 14 days from the ARD to complete this?

 

A:

No, the completion date for a Significant Change assessment is not based on the ARD. Instead, it is based on the date the IDT determined the assessment was needed. According to the RAI manual (page 2-25), after the IDT has made this determination, this initial identification of the significant change should be documented in the clinical record. This initial identification will serve as day 1 of 14 for the period for setting and completing the Significant Change assessment. While the assessment can be set on the same date as the initial identification, it must be set within the 14-day span and completed, including CAA(s), on or before day 14 (page 2-26). It should be noted, as Significant Change assessments are required for hospice election and revocation, the census line in the clinical record can serve as documentation indicating day 1 of the 14-day span.
