For the Quality Measure “Percent of Residents Who Newly Received an Antipsychotic Medication (SS),” is the initial assessment used in the calculation always an Admission or 5-Day?

 

A:

For this measure, the initial assessment is the first assessment completed after the Entry Tracking Record. In some situations, a Discharge assessment may be the initial assessment. For example, a resident who admits with an order for an antipsychotic medication and subsequently discharges soon after admission may trigger this measure if he/she was not in the facility long enough to receive a dose of the antipsychotic medication and returns to the facility with antipsychotic administration reported on a subsequent assessment. The Discharge assessment (which may be combined with the PPS 5-Day assessment for Medicare beneficiaries) would be the initial assessment.
