Q:

My facility is triggering high for the Quality Measure: Prevalence of Behavior Symptoms Affecting Others, but we don’t have many behavioral issues in our population. Why is this happening?

 

 

A:

This measure may trigger if the most recent MDS is coded with a 1, 2, or 3 for behavior frequency on the following items:

 

  • Physical behavioral symptoms directed towards others (E0200A)
  • Verbal behavioral symptoms directed towards others (E0200B)
  • Other behavioral symptoms not directed towards others (E0200C)
  • Rejection of care (E0800)
  • Wandering (E0900)

 

It’s important to ensure MDS items are coded accurately. Double-check any fields that may auto-populate from point-of-care documentation. Additionally, pay close attention to the coding for “Rejection of Care” (E0800). The RAI Manual specifies that rejections based on the resident’s preferences or goals should not be coded. Staff training is essential to prevent miscoding of these items.
