Q:
One of the residents at my facility was recently put onto a fluid restriction for ESRD, what should I be concerned about for documentation and care planning?
A:
Fluid restrictions, and any dietary restrictions, are not uncommon, but can present some potential documentation pitfalls. First, the restriction itself should be documented including the indication for the restriction. In the example above, this would be documentation of the fluid restriction related to ESRD. Second, resident awareness and consent should be considered and documented. The resident should be educated regarding the need for the restriction, and the restriction should be consistently followed while respecting the resident’s personal preferences. Preferences and restrictions should be reviewed with each care plan meeting to ensure they are up to date and being followed. Third, there should be a system in place to ensure staff are aware of the restriction and to track compliance. Communication across the IDT with monitoring are essential to ensure adherence to the prescribed fluid restriction. For example, a resident with a 1,500 mL/day fluid restriction may have 720 mL allotted with meals and nourishments provided by dietary (240 mL per meal), up to 360 mL available during medication passes (120 mL per shift), and up to 420 mL available as bedside fluids. Staff should document the amount of fluids provided and consumed from each source and monitor cumulative intake throughout the day to ensure compliance with the restriction. Finally, the care plan should reflect the ordered restriction, resident preferences, and interventions in place to assist staff in following the restriction.
