Q:

One of our long-term residents fell and fractured their femur requiring a hip replacement. They returned to our facility on a skilled stay. The IDT met to discuss a primary diagnosis. Therapy insists we should use Z47.1 Aftercare following joint replacement surgery, but some of the available hospital documentation uses S72.141A Displaced intertrochanteric fracture of right femur, initial encounter for closed fracture. Which is the correct code?

In this case, neither is precisely correct, but the S72.141A is closer.

The ICD-10-CM Official Guidelines for Coding and Reporting FY26 addresses the coding of fractures related to injuries and the use of aftercare code as noted below:

The aftercare Z codes should also not be used for aftercare for injuries. For aftercare of an injury, assign the acute injury code with the appropriate 7th character (for subsequent encounter).” Additionally, as noted on page 78 of the guidelines, “The aftercare Z codes should not be used for aftercare for conditions such as injuries or poisonings, where 7th characters are provided to identify subsequent care.” In essence, the 7th character serves the same purpose as the aftercare Z code in care of injuries. In this example, the acute injury code is the fracture code of S72.141.

The next step is to determine the appropriate 7th character.  The hospital paperwork uses “A.” This indicates an “encounter where the patient is receiving active treatment for the condition” (p. 78). This is the correct code for the hospital as this is the site of initial treatment. Your SNF is providing subsequent care which includes rehabilitation following the initial treatment. For this the “D” code should be used indicating “the patient has completed active treatment of the condition and is receiving routine care for the condition during the healing or recovery phase” (p .78). The appropriate primary diagnosis in this situation is S72.141D.

Therapy may be thinking the aftercare code Z47.1 is necessary in order to classify into the major joint replacement clinical category, but this is not accurate.  Correctly assigning the fracture code as noted above in section I0020B of the MDS and coding the surgically procedure of a hip replacement in section J2310 (hip replacement total or partial) will result in the major Joint replacement.
