Q: A resident had a fall at home resulting in multiple fractures. The resident was admitted to our facility following hospitalization and surgery. Would orthopedic aftercare be an appropriate primary diagnosis?
A:
Following a traumatic fracture, an orthopedic aftercare Z code is not used to represent the healing fracture. When a traumatic fracture is receiving routine care during the healing or recovery phase following active treatment, ICD-10-CM guidelines direct us to continue using the appropriate fracture code with the applicable 7th character for a subsequent encounter, such as “D” for routine healing when appropriate.

- Orthopedic aftercare: An orthopedic aftercare Z code may be appropriate for certain orthopedic conditions or procedures when the care is not directed at a healing traumatic fracture.

- Multiple fractures: Assign the appropriate ICD-10-CM code for each fracture being treated. The primary diagnosis should represent the condition chiefly responsible for the resident’s SNF admission and skilled care, based on the clinical circumstances and applicable coding guidelines. Fracture specificity: Ensure the fracture codes accurately reflect the documented site, laterality, fracture characteristics, encounter type, and healing status.
- PDPM clinical category: Ensure qualifying surgical procedures are accurately captured in MDS Section J when applicable. Under PDPM, the ICD-10-CM code reported in I0020B determines the resident’s initial clinical category, and certain qualifying surgical procedures reported in Section J may affect the final clinical category used for the PT and OT components.
PDPM and MDS Impact: Accurate ICD-10-CM diagnosis selection, sequencing, specificity, and MDS coding are important to accurate Patient-Driven Payment Model (PDPM) classification and facility reimbursement.
